Actor Consent Form
This form will allow for any people participating in the production to participate on the following dates (25/1/08 – Afternoon) (1/2/08 – all day).
hereby declares that they agree to be part of the acting team in our (Matthew Godwin, Ashley Welch, Adam Jackson) AS Media production of the Calcium Kid and are aware of what it may entail. They must also be aware of any health and safety issues and must abide by the regulations. Dates and conditions may be subject to change.

Teacher consent – the dates and times of this production may interfere with the above participants lessons, in the event of this we ask that they be excused from their lessons for the required time.Therefore we would ask that you sign below to state that you agree to the participants absence from your lesson on the stated times and dates in order to be involved in our production.
